Bad form

Laura was just finalizing the application for the next school year scholarships. The form is due tomorrow (today, now). There is a series of questions for which the applicant has to check a yes or no box. (Example: Are you a single father with good family values?) The only problem, the columns of check boxes are not labeled with yes or no. She labeled the boxes herself.
